PHOTO PERMISSION FORM

I give Sue Conant permission to take pictures, and videos of my

Child/children _______________________________.  These

photos may be posted at Sue’s Child Care, put in yearbooks to

be distributed to other families, used in projects, or put in the

newspaper.  They may also be used as examples in projects and

presented in a class I teach to other providers.
